What is Sciatica?

Sciatica describes pain and neurological symptoms that follow the path of the sciatic nerve; typically from the low back or buttock down the back of the leg. Symptoms vary in intensity and may include:

  • Sharp, shooting or electric-type pain that radiates below the knee.

  • Numbness, tingling, or pins-and-needles in the leg or foot.

  • Muscle weakness (for example, difficulty lifting the foot) or altered gait.

Common causes we address at our clinic include:

  • Lumbar disc herniation or bulge compressing a nerve root.

  • Lumbar spinal stenosis (narrowing of the spinal canal).

  • Neural tension from muscle entrapment such as piriformis syndrome or pelvic dysfunction.

When to seek urgent care: if you have new bowel or bladder changes, saddle anesthesia (numbness in the groin), or rapidly progressing leg weakness, go to the ER or contact your physician immediately.

Physical therapy’s role: we perform a targeted exam to differentiate nerve-root involvement from other sources of leg pain, prioritize pain control, restore mobility, and retrain movement patterns to reduce recurrence. We coordinate with your physician when imaging is needed.

We treat sciatica by combining a targeted clinical exam with hands-on care and progressive movement training. Our goal is to reduce your pain, restore nerve and joint mobility, and retrain the muscles that support your spine and pelvis so the problem doesn’t keep coming back.

How we assess you

  • Detailed history: onset, pattern, activities that help or worsen symptoms, prior imaging/surgeries.

     

  • Focused neuro-musculoskeletal exam: strength, reflexes, sensory testing, straight-leg raise and slump tests (neurodynamic screening), hip and lumbar mobility, and functional movement analysis.

     

  • Differential screening to identify whether symptoms are nerve-root, peripheral entrapment (piriformis), or referred pain.

What to Expect from Jennifer Klein Physical Therapy:

As Physical Therapists, we work to restore function, improve mobility, and decrease pain with the goal of reestablishing a patient’s prior level of function.

We will evaluate, identify objective findings, make an assessment, & develop a treatment plan for each patient on an individual basis.  Objective testing may include some of the following categories: motion, strength, posture, reflexes, muscle tension, joint mobility, balance, agility, coordination, & special tests pertaining to your symptoms and or diagnosis.

We develop a treatment plan specific to addressing the pertinent objective findings. Each patient’s individual treatment plan is geared toward reaching specific goals that are set by the Physical Therapist & the patient together. The treatment plan may include traditional PT modalities, such as ultrasound, electrical stimulation, heat/cold therapy, as well as homeopathic approaches such as cupping & dry needling. Manual therapy such as joint or soft tissue mobilization may also be included. Exercises may be incorporated to address weakness, tightness, instability, lack of balance, & coordination; both traditional PT exercises and exercises utilizing Pilates equipment.
