Please feel free to schedule online, reserving the time slot that best accommodates your schedule. If you plan on filing with your insurance company, we will need 24 hours in advance, the front and back photos of your insurance card and your date of birth in order to verify your benefits and inform you of your financial responsibility before your scheduled appointment. If you opt to use your HSA or Flexible Spending card or go straight cash pay, please follow up with us with a call or email us to determine your cost.
